Make a Referral
Book your assessment first. Once your booking is confirmed, you’ll be directed to complete the secure referral form so we can gather the information needed for the appointment.
Secure Document Upload
To protect personal data, document uploads are password-protected and encrypted via Microsoft 365. Please contact our office or check your referral instructions for the upload password.
